When can I come in for a medical appointment?

Our appointment hours are:

Monday:              7:00am-4:30pm

Tuesday:              7:00am-4:30pm

Wednesday:        7:00am-4:30pm

Thursday:            7:00am-4:30pm

Friday:                  7:00am-4:30pm

How do I transfer medical records from my previous practice?

Upon receipt of a signed release form, your former practice will forward your records to our office.  It is best to contact your former office for their form, since their release policies and prices will be disclosed to you.  We usually do not need all of your records, generally the last couple of years will suffice, unless you have a complicated medical history.

Which health insurance carriers are accepted?

FPSR participates with most major insurance carriers including:

  • Anthem Blue Cross Blue Shield
  • Aetna
  • Cigna
  • Humana
  • Medicare (We do NOT accept Medicaid nor Healthkeepers PLUS)
  • Optima
  • United Healthcare
  • Tricare (We do NOT accept Tricare Prime)

There are many plans within each of these carriers and because we aren’t always informed of these new plans, it is highly recommended that you call your insurance carrier to verify that we participate. These participation agreements can change frequently and we want you to have the most accurate information.

What do I need to bring to my visit?

For your visit with us, be sure to bring the following:

  • Driver’s License
  • Insurance cards (including any drug coverage cards)
  • All medications
  • Any records from former doctors that you may possess

What forms do I need?

For your convenience, we are now a paperless office.  All new patient forms are completed on touch-screen tablets upon your arrival.

I don’t have insurance. Can I make an appointment?

Yes!  We see patients on a self- pay basis.  Payment of $180 is collected at check-in for new patients and $99 for established patients. Any remaining balance will be collected after the visit.  If you are having a complete physical as self-pay, please call our billing office to discuss pricing.  804-330-5984

I am running late for my appointment. What do I do?

We ask that you call the office and let us know what time you will arrive.  We will check with the providers to see if you can be seen at that time or if it would be better to reschedule.

What happens if I don’t show up for my appointment?

We expect you to call us to cancel your scheduled appointment at least 24 hours in advance so that we have a chance to offer that time slot to another patient.  From time to time, we realize that calling in advance is impossible, however, repeatedly not showing up for appointments could result in being assessed a fee of $150 for an extended appointment like physicals or office surgeries and $50 for regular appointments.

How do I get my lab results?

Results are delivered to the patient portal and patients can view the results on their computer by logging in to their portal account.  Portal is the fastest way to receive results.  For our patients who cannot access the portal, we will discuss your results at your follow-up appointment or mail results to your address of record.  If results are abnormal or of an urgent nature, patients will be contacted by phone.

How do I get my refills?

Typically, our patients are given enough of their medication until their follow-up appointment.  When patients need refills between visits, we ask that they contact their pharmacy and have the pharmacy request the refill from us via fax.

What happens if I need to be seen over the weekend or a holiday?

If a patient is having a life threatening medical emergency, then they should call 911 or proceed to the closest Emergency Room or Urgent care facility.  FPSR also has an “after-hours” on call service to patients needing guidance after hours.

How do I prep for a complete physical?

Please arrive having fasted (nothing but water) for at least 12 hours prior to your appointment.  We do encourage patients to drink at least 8oz of water before coming in.  Patients may take their regular medications as well, unless those medications need to be taken with food.  Please do not use body lotion.
